One dead in Jaya supermarket collapse (Updated)




PETALING JAYA, 28 May 2009: Part of the building which once housed the popular Jaya Supermarket in Section 14/20 collapsed this evening, killing one worker and trapping six others.

Two others were rescued and sent to the University Malaya Medical Centre (UMMC), Petaling Jaya OCPD ACP Arjunaidi Mohamad said.

All the nine victims of the 5pm incident were Indonesian workers.

The workers, who were preparing the building for demolition, were at the ground floor of the six-storey building at the time of the incident.

As of 7pm the fate of the six trapped workers were still unknown, he said.

"Search and rescue operations are underway and we have brought in sniffer dogs to locate the victims," he said.

Among those involved in the operation are 50 police personnel, fire and rescue (30), Smart (15), Civil Defence Department (30) and Rela (30), he said.

Housing and Local Government Minister Datuk Seri Kong Cho Ha said the Fire and Rescue Department was informed of the incident at about 5.08pm and arrive at the scene three minutes later.

When contacted, Civil Defence Department officer Lt Shahrul Kama Mohd Johari said the department had dispatched six ambulance cars.

UMMC deputy director (Professional) Prof Dr Jamiyah Hassan said the dead was known as Maskor, 28, while the injured were Maskor's brother Suryano, 32, and Sale, 48.

"The two injured victims are in stable condition," she said.

Jamiyah said 45 paramedics from three hospitals - UMMC, Sungai Buloh Hospital and Kuala Lumpur Hospital - were at the scene of the incident to provide medical assistance.

Housing and Local Government Minister Datuk Seri Kong Cho Ha said the Fire and Rescue Department was informed of the incident at 5.08pm and arrived at the scene three minutes later.

When contacted, Civil Defence Department officer Lt Shahrul Kama Mohd Johari said the department had despatched six ambulance cars.

An eyewitness Syed Mohammad Aidid Syed Ahmad Aidid said he heard a loud roar like that of a thunder while driving pass the area.

"I saw dusts rising in the air and pieces of stone came crashing down on my car like a hailstorm," he told Bernama.

Syed Mohammad Aidid, 41, said he had just left the nearby Digital Mall building to go to his office at Phileo Damansara when the Jaya Supermarket building collapsed.

"I saw in front of me several cars but they managed to escape. I just floored the accelerator but some of the rubble hit my car and dented it.

"I was shaken with fear and prayed that I would be out of harm's way," he said. — Bernama

Supermarket Runtuh Di PJ






PETALING JAYA – Operasi mencari dan mengesan enam mangsa yang tertimbus dalam runtuhan pusat beli belah Jaya Supermarket di Seksyen 14 di sini masih diteruskan sehingga malam ini.Sehingga pukul 9 malam ini, seorang buruh warga Indonesia yang meninggal dunia dalam runtuhan itu dikenali sebagai Maskor, 28.Mangsa yang cedera pula dikenali sebagai Suriono, 31, dan Salleh, 48. Suriono kini dirawat di Wad Kecemasan Pusat Perubatan Universiti Malaya (PPUM) manakala Salleh dirawat sebagai pesakit luar.Ketika kejadian, dianggarkan terdapat 20 hingga 30 orang pekerja warga Indonesia berada di bangunan tersebut.

SAFETY TIPS – PERSEKITARAN SELAMAT

SEWAKTU hidup sebagai suami isteri, banyak
peralatan dan perhiasan dalam rumah boleh diletakkan
di mana saja yang anda inginkan.

 Namun, apabila bilangan keluarga bertambah dengan
kehadiran si kecil,anda mula perlu mengalihkan
peralatan yang mungkin boleh membahayakan
keselamatan anak.

 Bermula daripada anak mula belajar mengengsot
menggunakan dua tangan dan punggung, merangkak
dan berdiri, peralatan dalam rumah yang boleh
membahayakan akan dialihkan beransur-ansur hingga
keadaan rumah jauh berbeza berbanding ketika anda
memasuki rumah untuk memulakan hidup berkeluarga.

 Tiada lagi pasu bunga, benda tajam atau barangan
mudah pecah yang diletakkan di lantai atau tempat
yang difikirkan boleh dicapai oleh anak. Malah, ada ibu
bapa yang mula memikirkan untuk tidak memakai alas
meja kerana khuatir ia ditarik anak menyebabkan
barangan di atasnya pecah atau berselerak.

 Persekitaran yang selamat diperlukan untuk anak
bergerak dengan mudah.Pelbagai kemungkinan boleh
berlaku pada anak anda akibat kecuaian seperti
terhantuk pada bucu tajam, termakan benda kecil,
kotor atau yang membahayakan, juga terkena benda
panas atau tajam.

 Langkah pencegahan dan sikap berhati-hati perlu ada
dalam diri ibu bapa bagi mengelak kejadian tidak
diingini menimpa anak anda.


Berikut adalah panduan untuk ibu bapa menyediakan
persekitaran yang selamat untuk bayi.;


 Pasang pagar pada tangga atau tempat kurang selamat,
misalnya pintu ke bahagian dapur. Pagar khusus itu biasanya
dijual di butik bayi atau pasar raya.

 Rumah biasanya dipenuhi pelbagai barangan dan
peralatan yang boleh membahayakan kanak-kanak. Simpan
bahan beracun di tempat khusus, manakala benda tajam
seperti peralatan dapur disimpan di tempat yang tidak mudah
dicapai bayi.

 Elak meletak air minuman panas di meja kopi tanpa anda
memantau pergerakan si kecil. Kelalaian boleh menyebabkan
kemalangan pada anak iaitu apabila dia menarik alas meja
atau mencapai cawan hingga menyebabkan kulit melecur.

 Jangan letak benda panas seperti cerek, periuk atau
seterika di lantai atau tempat yang mudah dicapai anak.

 'Walker' hanya digunakan ketika anak dalam pengawasan
anda, bukan sepanjang masa. Pantau pergerakan anak
supaya dia tidak menggunakan walker untuk mendekati dapur
atau benda panas.

 Sentiasa kunci pintu bilik air dari luar bagi mengelak anak
kecil memasukinya. Jangan biarkan baldi atau besen
mengandungi air kerana jika terleka, anak anda mungkin
bermain air yang boleh menyebabkannya lemas.

Hari Pertama Sebagai SHO....Zahid


Mula2 sekali saya nak ucapkan tahniah pada Zahid kerana telah berjaya dan temuduga sebagai safety supervisor tempoh hari dan jangan putus asa untuk Zarin kerana tidak berjaya. Esok Zahid akan memulakan tugasnya sebagai Safety supervisor di kilang Nestle shah alam. kerja2 renovation kat kilang Nestle. saya mendoakan agar Zahid dapat menjalankan tugasnya dengan cemerlang dan dapat mengunakan segala ilmu2 yang dipelajari dalam bidang safety kedalam tugas sehariannya untuk menjaga keselamatn pekerja di tempat kerja nya.

Untuk Zarin jangan putus asa cuba lagi, munkin kali ini tak de rezeki cuba lagi lain kali

Interviu Kerja

Semalam aku bawa Zahid dan Zarin pegi interviu untuk jawatan Safety supervisor di Subang Jaya. Semalam ada dekat2 10 orang pengamal keselamatan dan kesihatan pergi bersama-sama untuk interviu. Sebahagian darinya telah bekerja dan mempunyai pengalaman dalam bidang safety. Terdapat banyak persaingan kat situ, harap2 Zahid dan Zarin diberi peluang untuk melangkah kebidang ini. Ini merupakan interviu pertama untuk Zahid dan Zarin dalam bidang safety. Mengikut SMS yang aku dapat dari kawan aku gaji yang ditawarkan agak lumayan iaitu basic sekitar RM 3000 + allowance + ot boleh cecah 4 ~ 5 Ribu sebulan tapi base pada exprience. Aku tak kisah sangat janji Zahid dan Zarin dapat jawatan tu ok la, dapat la mereka mengembangkan skill mereka dalam bidang safety.

Project aku kat KL Plaza dah hampir nak siap, kalau ikut jadual asal sepatunya hujung bulan April hari tu dah siap tapi atas sebab-sebab teknikal telah dilanjutkan sehingga hujung bulan ni dan kontrak aku sebagai safety supervisor akan tamat pada hujung bulan ini, dan kalau ada rezeki aku akan sambung lagi kerja kat KL Plaza tu tapi dgn Contractor lain pulak. Ada 2 company telah panggil aku interviu untuk jawatan safety supervisor kat situ. Projek Fasa 2 ini memakan masa lebih kurang satu tahun dan kalau dapat boleh le buat duit sikit sebab yang ni offer dia lebih sikit he he he....mesti la demand sikit sebab dah ada experience. Aku juga bercadang untuk ambik exam pada bulan Ogos nanti dan aku akan ambik course utk scaffolding sebab aku duduk dlm bidang construction, ada sijil scaffolding lagi cantik dan lagi masyuk

Contractor Safety Agreement

This safety agreement shall be presented to all contractors who perform services on any XXXX owned, leased, or otherwise controlled premises. A signed copy of this agreement and a certificate of insurance must be received before the contractor begins performance of the contracted service.

Contractor Safety AgreementFOR CONTRACTORS PROVIDING SERVICES ON XXXX OWNED, LEASED, OR OTHERWISE CONTROLLED PREMISES

This Agreement has been drafted and published to help prevent accidents and personal injuries. It is not intended to be all inclusive, but rather a guide for the contractor. It is the responsibility of the contractor's management personnel to enforce these and all other safety rules and good safety practices. The contractor shall provide the necessary safety equipment and perform the required services in such a manner as to eliminate the cause of personal injuries and accidents. All XXXX Safety Rules must be obeyed when working on Vertis-owned, leased, or otherwise controlled premises.


1.0 Occupational Safety and Health Act

1.1 All materials supplied to, or work performed on, XXXX premises shall comply fully with the applicable provisions of federal and applicable state occupational safety and health acts, standards, and regulations covering Lockout/Tagout, Hazard Communication, Lift Truck Safety, Confined Spaces, Hazardous Materials, Flammables and Combustibles, and other applicable provisions, acts, standards, and/or regulations.
1.2 The contractor hereby certifies that all such materials and work performed will conform to and comply with such standards and regulations.

2.0 Personal Protective Equipment
2.1 The contractor at his own expense shall provide personal protective equipment for his employees.
2.2 All contractor employees must, while on XXXX owned, leased, or controlled premises, wear required safety equipment whenever engaged in work requiring personal protective equipment.

3.0 Protective Helmets
3.1 Contractor employees must wear protective helmets when working in areas where there is a possible danger of head injury from impact, falling or flying objects, electrical shock, or burns
3.2 The contractor is responsible for providing, posting, and observing required signage in such areas.

4.0 Welding and Welding Equipment
4.1 Contractor personnel must obtain written permission from authorized XXXX Plant Engineering or Maintenance Department personnel before welding or cutting.
4.2 A shield must be used to protect all persons from the flash of electric welding whenever possible.
4.3 All electric welding machines are to be equipped with standard electrical equipment to fit the outlets existing on XXXX premises. Temporary electrical hookups will not be permitted unless made by a qualified technician.
4.4 Acetylene and oxygen cylinders shall be protected against tipping by use of standard portable welding carts to which the cylinders must be secured. If portable welding carts are not immediately available, the cylinders shall be secured to a stationary object of sufficient strength to hold the cylinder until a cart can be obtained. Unless a recess in the cylinder protects the valve, a metal cap shall be used to protect the valve against damage when the cylinder is not connected for use. The proper equipment to shut off the valves in case of emergency must accompany each set-up.
4.5 When not in use, acetylene and oxygen cylinders must be removed to an approved outside storage area.

5.0 Ladders
5.1 All ladders used shall be in good condition. When used in buildings, they shall be equipped with ladder safety shoes.
5.2 Contractors WILL NOT be permitted to use XXXX ladders.
5.3 It is prohibited to place a ladder in front of doors opening toward the ladder except when the door is blocked open, locked, or guarded. It is prohibited to place ladders on boxes, barrels, or other unstable bases to achieve additional height. When not in use, all ladders shall be safely stored in an OSHA-approved manner.

6.0 Construction
6.1 All excavations shall be provided with suitable solid barriers or railings plus adequate warning lights or other devices.
6.2 Necessary precautions shall be taken to protect all persons in any area when construction work is being performed overhead or at floor-level. Guards or other adequate protection must be provided whenever holes, inserts, bolts, or other hazards are left in, on, or above the floor.
6.3 Overhead construction work must never be done above any person or over any area where people and/or trucks are moving without first obtaining specific permission from the XXXX Plant Maintenance Supervisor.
6.4 Areas in which construction work is being performed must be kept clean, orderly, and safe. The contractor must remove combustible materials and other debris from the plant property at the end of each working day.
6.5 All equipment being used by the contractor such as hand tools, trucks, etc., must be kept in safe operating condition.
6.6 Loose materials such as bolts, nuts, or hand tools must not be left lying on beams, ledges, or any place from where they could fall or be knocked off, causing damage or injury. They must be immediately removed to the floor level.
6.7 Guards removed while making alterations or repairs on equipment must be replaced before the equipment is returned to production.
6.8 Written approval from the XXXX Plant Maintenance Department must be obtained before any explosives or tools utilizing a power cartridge can be used on XXXX premises. Contractors assume sole responsibility for bodily injury and property damage caused by use or possession of such explosives or power cartridges.
6.9 Any spilled materials, such as grease, oil, paint, etc., shall be immediately cleaned up.
6.10 All combustible scrap, debris, and waste materials (e.g., oily rags, etc.) must be stored in covered metal receptacles and removed from the worksite promptly. Proper storage to minimize the risk of fire and spontaneous combustion must be practiced.
6.11 Only approved containers and tanks are to be used for the storage and handling of flammable and combustible liquids. All connections on drums and combustible liquid piping, vapor, and liquid must be kept tight. All flammable liquids should be kept in closed containers when not in use.
6.12 No Smoking signs must be posted on liquefied petroleum gas tanks.
6.13 When an opening is made in the roof or walls of a building, the inside must be protected from weather until the opening is closed.
6.14 Any accumulation of dust must be routinely removed. Dust should be cleaned up with a vacuum system to prevent the dust going into suspension.
6.15 Metallic or conductive dust must be prevented from entering or accumulating on or around electrical enclosures or equipment.6.16 Smoking is expressly prohibited in smoking-restricted areas and any area used for the storage of flammables or combustibles.

7.0 Underground Service Hazards
7.1 Before starting any excavation, the Plant Maintenance Department must approve driving of objects into the ground or through floors, or the hauling or dumping of heavy loads, to ensure that no underground service such as power cables, sewer or fire lines, etc., are in the area. Hand excavation must be used to locate underground service in the area before proceeding with mechanical means.
7.2 Written clearance must be obtained from the Plant Maintenance Supervisor before shutting off any utility or service.
7.3 Written permission from the Plant Maintenance Supervisor must be obtained in writing before shutting off or placing out-of-service any fire protective device or service.

8.0 Leaving Job Before Completion
8.1 When it is necessary to leave a job before completion, all materials, rigging, boards, loose nails, and other debris must be carefully removed from the floor.
8.2 There shall not be materials or equipment left overhead or on the roof unless secured in place.
8.3 Adequate barricades and warnings must be erected at all openings, excavations, and obstructions.
8.4 If a crane is being used, the boom must be lowered to the ground prior to leaving the job.

9.0 Environmental Waste Management
9.1 The contractor is responsible for the proper removal of any and all waste or hazardous material used, generated, or produced as a result of the contractor's activities while on XXXX premises.
9.2 Prior to any hazardous waste generation, XXXX shall be provided with the:Name and location of the waste facility;EPA ID number of the waste facility; andState Approval Number of the waste facility if applicable.
9.3 XXXX reserves the right to reject any disposal facility that has any Class I violations or may be a Superfund site.
9.4 When a waste facility is approved, and immediately after shipment of waste occurs, XXXX shall be provided with:a) Copies of all Manifests or Bills of Lading; andb) Total Quantity Generated.

10.0 Security
10.1 All outside contractor employees will be required to submit to any security provisions in place for any XXXX employee under the same circumstances.

11.0 Communication
11.1 Business agents representing contractor employees must conduct their business in the visitors' lobby or at such places designated by the Facility Manager.
11.2 When contact with contractor employees is necessary, the Business Agent must first report to the receptionist and have the receptionist notify the Maintenance Supervisor in charge of the project.
11.3 The Maintenance Supervisor will notify the contractor's superintendent or supervisor who will have the specified contractor's employee report to the reception area for the conduct of business.
11.4 A designated daily time and place will be arranged. A designated XXXX Maintenance Supervisor will meet with the Contractor Project Manager. Items that must be discussed and documented are:a) Size of contractor crew for that dayb) The project progress, what is to be done today, and what was completed the day
before, status of pre-arranged timelinec) Safety inspection and discussion of concerns by the XXXX representative

12.0 Hazardous Communication
12.1 The contractor shall provide XXXX with copies of Material Safety Data Sheets (MSDS) for any products used on site.
12.2 The contractor shall have copies of Material Safety Data Sheets available at all times.
13.0 Completion of the Job
13.1 All scaffolding and unused materials shall be removed from trusses, roof, platforms, and floor to a designated place outside of the building as soon as their use is completed.

14.0 XXXX Property
14.1 Contractor personnel are not authorized to move any XXXX equipment (e.g., machinery, cranes, tractors, fork trucks, etc.). Only the XXXX Maintenance Supervisor has the authority to have such equipment moved.
14.2 Trucks or other vehicles being operated on XXXX property or premises must be operated safely at all times and stored in authorized places as directed by the Plant Maintenance Department.
14.3 XXXX reserves the right to inspect all equipment used by the contractor or any of its subcontractors, and to prohibit the use of any equipment judged to be unsafe.
14.4 XXXX reserves the right to stop any job being performed in a manner deemed unsafe by the Safety Department or Plant Department.
14.5 Proof of satisfactory insurance coverage - including liability insurance and workers' compensation insurance - must be furnished by the contractor and accepted by XXXX before any work is started.

15.0 Employment of Workers
15.1 The contractor are required to employ legal worker with valid working permit and properly insured.
15.2 The contractor shall submit list of his employees together with their particulars including CIDB card to Person-In-Charge.
15.3 The contractor shall update their list of employees from time to time.

16.0 Company CIDB Registration
16.1 The Contractor company shall be registered with CIDB for the purpose of carrying out the project.
16.2 In the event that you have not complied with the above undertaking, you shall be solely responsible for any legal action taken against you.


CONTRACTOR SAFETY AGREEMENT ACKNOWLEDGEMENTThe SAFETY AGREEMENT has been read and its conditions are hereby accepted by the undersigned on behalf of the Contractor and its employees, agents, subcontractors, and subcontractor employees and agents.The undersigned assumes full responsibility to inform its employees, agents, and subcontractors about this SAFETY AGREEMENT, and agrees that it will conform and will have all employees, agents, and subcontractors conform with this SAFETY AGREEMENT at all times while on the premises controlled by XXXX.
It is further understood that any person not conforming with the SAFETY AGREEMENT shall not be permitted to perform services on such premises.It is further understood by the undersigned that compliance with this SAFETY AGREEMENT is a continuing requirement and that in consideration of the undersigned accepting any contract to be performed on XXXX premises, the acceptance of and compliance with these conditions shall be automatically continued from job to job and year to year unless written notice revoking such acceptance is given by the undersigned to XXXX, to which this SAFETY AGREEMENT was originally submitted.

As additional, the Contractor are fully responsible for all liabilities/ consequences imposed by the authorities arising from not complying with this requirements. The Contractor have further agreed that a penalty and/or fine by XXXX will be imposed on you in the event that you have not complied with the above undertaking

Protection of oil spillage


Gambar di sebelah menunjukkan cara untuk mencegah daripada minyak tertumpah yang boleh menyebabkan pencemaran alam sekitar di tapak bina. Suatu pengalas besi yang berisi pasir telah direka khas semata-mata untuk menadah tumpahan minyak hitam dari mesin pembengkok besi. Apabila pengalas itu telah dipenuhi dengan minyak hitam, maka hendaklah pasir tersebut dibuang ke tempat yang sesuai dan diganti dengan pasir yang baru pula. Di samping itu, mesin hendaklah diselenggara dengan sekerap mungkin iaitu sekurang-kurangnya sebulan sekali.

Pelantar Kerja Untuk Pemasangan Cantilever

Gambar di sebelah menunjukkan suatu pelantar kerja (working platform) dipasang pada 'slab' luar untuk kerja memasang 'cantilever catch platform'. Pelantar kerja ini telah direka khas oleh jurutera bertauliah (Professional Engineer) untuk kerja pemasangan tersebut. Jumlah pekerja yang dibenarkan berada di dalam pelantar tersebut cuma seorang sahaja. Beban kerja selamat untuk pelantar ini adalah 100kg sahaja. Pekerja hendaklah memakai tali pinggang keselamatan dengan sempurna ketika berada di dalam pelantar tersebut.

a) Mengarahkan kakitangan kontraktor kecil di bawah kawalannya terhadap keperluannya melakukan kerja dengan selamat dan tanpa berhadapan dengan risiko yang diluluskan.

b) Menghentikan operasi kerja jika kecemasan dikenalpasti dan memerlukan pemeriksaan ke atas tindakan atau keadaan kerja yang tidak selamat sehingga perkara tersebut dikenalpasti dan diatasi.

c) Memastikan semua peralatan dan kelengkapan memenuhi spesifikasi yang perlu yang berkaitan dengan keselamatan.

d) Memastikan supaya perkerja-perkerja memakai dan menggunakan Peralatan Perlindungan Pekerja dengan baik berdasarkan keadaan kerja.

e) Memastikan standard kebersihan di tapak kerja dikekalkan dan membuat rancangan untuk pembuangan sisa kerja.

f) Menjalankan mesyuarat perkembangan kerja ( progress meeting )kepada kakitangan dan kontraktor dan memastikan kontraktor kecil yang mana para pekerjanya telah menerima sesuatu yang penting berkenaan tentang keselamatan dan kesihatan perkerjaan dari mesyuarat tersebut.
